Our decisions and actions assure a supportive environment for patients and staff

  • The following is a summary of the benefits offered to full-time employees (regularly scheduled for 30+ hours per week).

    • Medical, Dental and Vision insurance is available on the first of the month following 60 days.

    • Company-paid Life and AD&D insurance ($30,000) and Long-term Disability.

    • Six company-paid holidays.

    • 100 hours of PTO the first year (3.846 hours per pay period), 140 hours the second and third year (5.385 hours per pay period) and 180 hours (6.923 hours per pay period) thereafter.

    • PTO includes sick time in accordance with Oregon’s paid sick time law.

    • 401k Plan including a 50% company match up to 4% of earnings.

    • Paid Family and Medical Leave.

    • Employee Assistance Plan.

  • Moda Health of Oregon (3 Medical plan options available):

    • PPO $500 deductible

    • PPO $3,750 deductible

    • PPO $7,000 deductible

    • Vision is included in all medical plans.

    Moda or Willamette Dental (2 Dental plan options available):
